The workshop aims to highlight the challenges faced in treating older patients and to present existing solutions. Attendees will receive evidence-based tools and proven strategies for addressing key geriatric syndromes. The discussion will also outline a path forward in managing these issues. The workshop will begin with a case study of Ms. T, a fictional 80-year-old patient who has been referred to the rehabilitation department following a hip fracture. Ms. T presents a range of complex problems that the rehabilitation team must address within the context of the International Classification of Functioning, Disability, and Health (ICF). The last topic will discuss Age Friendly Care initiative in the US and implementing 4M (mobility, medication, mentation, and what matters) framework in inpatient rehabilitation setting. Vast majority of patients in inpatient rehabilitation unit/hospital are older adults with an average age of 69.3. However, few IRH/IRUs have specialized geriatric rehabilitation programs or access to a geriatrician. Age-Friendly Health Systems (AFHS), designed by the Institute for Healthcare Improvement (IHI) in collaboration with other organizations committed to improving the care of older adults, aim to follow evidence-based practices to improve the quality and safety of care and align care with what matters to older adults and their families.
