Spasticity after upper motor neuron lesions is of particular concern for PRM specialists. Although its positive effects can be advantageous for the rehabilitation of patients, it does need to be modulated to minimize its negative effects and complications. Botulinum toxin is commonly utilized for chemodenervation of spastic muscles in countries where it is readily attainable. Its effects are reversible, but its relatively high cost can limit access for repeat use in some circumstances.
Phenol, as an alternative option, is considerably cheaper and can also be very effective for Motor Point Blocks to provide a similar clinic effect on spastic muscle.
The use of ultrasound and the Modified Heckmatt scale can help to grade spastic muscle fibrosis in real time and assist in correctly choosing the target spastic muscles for administration of phenol. Success can be further enhanced by utilizing ultrasound guidance for identifying optimal intramuscular locations for phenol motor point blockade.
